Maybe the default behaviour changed.
You can use lxappearance as described in the bug report. Or edit the appropriate files.
Unless you have to do this after every upgrade I'd say it's a fix.

The Fedora maintainer has submitted a fix for this.

Install custom gtkrc to enable gtk-menu-images by default
That's what I said essentially.

It was a bug and it has since been fixed.

Have a go at changing symbol subjects to something different at that point back to breeze, take a stab at erasing your symbol store, attempt another client or expelling kde records in .reserve. I had this issue and attempted the abovementioned (had another bug that required expelling the documents) and one of those fixed it.
